A 2025 review in the journal Obesity Pillars changed that. It is the first published attempt to put actual doses on the supplements that matter for GLP-1 users, from protein to creatine, and it is worth reading closely before you spend money on a cabinet full of pills.

Why GLP-1 users need a supplement plan at all
These drugs work by cutting how much you eat, in some cases by up to 40%. That is the whole point, and it is why they work so well. But eat far less food and you take in far fewer of the vitamins, minerals and protein your body still needs every day. The weight comes off. So, quietly, can the nutritional foundation underneath it.
The bigger concern is muscle. When you lose weight on a GLP-1 medication, a large share of what you lose is lean tissue, not just fat. The review cites estimates that 20 to 50% of the weight lost can be lean body mass, and quotes a striking comparison from the field: losing 10% of your muscle over 68 weeks on one of these drugs is roughly what two decades of normal aging would do to you. - Omega-3 fatty acids: at least 1 g per day. Included for their anti-inflammatory role and possible support of muscle protein balance during rapid weight loss.
- Fiber: more than 10 g per day, for at least four weeks. Helps with the constipation many GLP-1 users get from slowed digestion, and feeds a healthier gut.
- Probiotics: 1 to 30 billion CFU per day. Aimed at supporting the gut microbiome and easing the digestive side effects that come with delayed stomach emptying.
- Curcumin: 500 to 1,500 mg per day. Included as an anti-inflammatory option, though the evidence here is the softest of the group.
The review also flags a longer list of micronutrients that commonly run low on GLP-1 medications and may need covering through diet or a supplement: calcium, iron, magnesium, potassium, vitamin B12, vitamins A, C and E, and choline. The caveat that matters most
Before you build a stack around these numbers, read the authors' own warning, because it is the honest core of the whole review. In their words, no clinical trial has yet directly evaluated dietary supplementation alongside GLP-1 treatment. Every dose above is extrapolated from research in general obesity and weight-loss populations, then applied to GLP-1 users by informed judgement.
That is not a reason to ignore the protocol. It is the reason to read it correctly. This is the best-reasoned starting point that currently exists, assembled by people who study sports nutrition and metabolism, but it is a set of educated recommendations, not proven fact. The direct trials, testing whether taking creatine or HMB or a specific protein target while on semaglutide or tirzepatide actually changes outcomes, still have not been run. Initial results from the wider literature are encouraging. They are not the same as a GLP-1-specific answer.
The vitamin D figure is where this caution bites hardest. Around 7,100 IU a day is well above the 4,000 IU daily upper limit set for the general adult population. That number reflects correcting a real, measured deficiency, not a dose everyone should swallow by default. How to use this without overdoing it
A protocol is only useful if it fits real life. Here is how to apply it sensibly rather than turning your kitchen into a pharmacy.
